Obtain Patient Certification Form
Meet with a registered physician, physician’s assistant, or nurse practitioner to discuss if the use of medical marijuana is appropriate for your condition. If the practitioner agrees you may benefit from medical marijuana, he/she must issue you a signed patient certification form.

Submit Required Documents
Create an account at the NY Government Website and click on “Health Applications” then “Medical Marijuana Data Management System” in order to access the online registration form. Note that some practitioners may be willing to assist you by submitting the registration form for you if it is included in their consultation fee.
Proof of Residency
You must provide proof of identity and NYS residency. Acceptable forms of proof include a NYS DMV ID or a recent photograph and a lease agreement, utility bill, or doctor/hospital bill.
Patients can designate up to two caregivers. A caregiver is a trusted individual you select who will be permitted to make dispensary purchases on your behalf if you are unable to visit in person. Once your application is approved, your caregiver(s) will need to register through the online system.
THE DEPARTMENT IS CURRENTLY WAIVING THE $50 APPLICATION FEE FOR ALL PATIENTS AND THEIR DESIGNATED CAREGIVERS.
Once you’ve submitted your registration and it has been approved, you may access a temporary registry ID card or wait approximately 7–10 business days to receive your medical marijuana registry ID card in the mail, at which point you can purchase medical marijuana from a qualified New York state dispensary.
